Referral system implementation between physical therapy and local YMCA

Abstract
Premier Health Upper Valley Medical Center partners with YMCA locations in both Piqua and Troy Ohio to provide athletic training services to members. This has helped create a community health program where physical therapy patients are referred to YMCA athletic trainers. Patients receive a free one-month membership to continue care with the athletic trainer once they have reached the end of their insurance-allowed visits or have been. This program provides the patient with access to care that they may not otherwise receive.
The goal of this project is to analyze barriers to patient care within the current referral system and improve the efficiency of both the program and common practices within the organization. Stakeholders have also provided feedback on considerations for improvement within the system that allow for better interprofessional communication.
